Updated IIP format

The IIP format has been improved and the new format is version 4.

The description part of the IIP format has been updated slightly to be well-formed XML. Previous versions of the IIP format are now deprecated. It is recommended that IIPs are updated to the latest version (even though the previous 2 versions are still supported). A script called convert_description_v3_v4 is included in the SDK and can provide a baseline for the update. The automatic conversion is rather crude though, so the result should be manually reviewed.
